This form should be filled out only if you will be requesting the use of CAC equipment in Latino Cultural Center, BSU Lounge, or Rainbow Lounge.

This form is only a request form and upon being submitted will be reviewed by CAC staff to determine equipment availability. You will be contacted once availability has been determined.
